• the service being provided by Axe Health in accordance with the Wellness Services Terms and Conditions.
• Axe Health (as part of DBG Health) collecting and using my personal information for the purpose of delivering clinical and administrative medical services with health care professionals, without which we couldn’t do so. I agree that that my personal information may be disclosed to third parties engaged by DBG Health to support in providing such services, including Health Engine, Helix and Dynamics 365. I understand that DBG Health’s Privacy Policy further explains how DBG Health collects, uses and discloses personal information and how I can access, correct or complain about the handling of my personal information.
